Matching Grant Funding Available for Daviess County
Pre-eligibility screening questions:
- Please confirm the legal entity name of the qualified 501c3 whose primary purpose is providing the direct essential health and human services within the proposed project space that the community will enter to receive services.
- Please provide your most recent 990 and the current year operational budget and year-to-date actuals, showing a breakout of sources of revenue and expenses.
- Please confirm the project address and the entity on title of the property.
- Please confirm there is no debt on the property, it is not used for collateral on anything else, and will not be in the future.
- Please provide a brief description of the project, new construction, renovation, expansion, etc. including SF, land acreage, what services the space is used for, why you are pursuing this project, etc. Site plan, floorplan, or other information is appreciated.
- What are the total project costs? Please include a detailed breakout, both details of the capital project budget and also any fundraising or operational reserve costs outside of eligible capital project costs.
- What is the SCPI Grant requested match amount (cannot be more than 50% of total eligible project costs)? ($50-200K requires one committee review, $200K-$1.5MM requires two committee reviews, all awards will have conditions of approval at prelim approval if applicable, at final approval, prior to transmittal of funds, and post funding)
- Where will the other funding sources for the match come from with estimates and timing? Or if you have already raised the match, please note that. If you aren't sure, please make your best estimate. If any funds for this project are indirectly or directly funded by Lilly Endowment Inc., please note the specific grant and amounts, and whether it is projected, applied/pending, awarded, or received. The same match cannot be applied to multiple match requirements for different awards. Feel free to transmit award letters with details when funding is from Lilly Endowment Inc.
- Please share the status of construction drawings, executed contracts, and finalized costs, along with timing for design/engineering and construction and completion. Please note that SCPI grant awards are funded toward the end of the project after reconciling costs and paid to date at the time of request. Completed projects cannot apply.
The following are NOT eligible:
- 501c3 whose primary purpose is NOT providing essential health and human services directly to the community (ie, primary purpose of entity on title is owning property or fundraising)
- 501c3 established by and governed by its related for-profit entities or other ineligible entities
- Real estate developers
- Economic Development Corporations
- Community Development Corporations (quasi-government entities)
- Government entities
- Public education foundations and corporations
- Market-rate sports and recreation facilities
- Outdoor recreational space/trails
- Museums/Theaters/Art Galleries
- Cultural Centers
- Libraries
- Humane Societies and other pet related organizations
- 501c3 Hospitals/Medical Centers that are not primarily discounted and free services
- Religious organizations (must have a separate 501c3, file annual 990s, with separate governance and authority, including seats with primarily local community members who are not affiliated with the religious organization)
